Each year, we receive a limited number of paper tax forms. The government does not tell us when we will get which forms.
Whatever paper forms we have are available at the library near the front windows on the east side of the building. As you enter the library's inner front doors, turn right immediately.
Other forms are available online, and may be printed at the library for 10¢/side. Ask us to print them, use one of our public computers to print them, or print them out at the library from your own mobile device or computer.
Do you want tax forms mailed to you?
Federal forms and booklets can be requested by calling 1.800.829.3676.
State tax forms and instructions can be requested by mail: 608-266-2772 or DORFormsRequests@wisconsin.gov.

- The Federal forms are available at U.S. Internal Revenue Service.

Tax forms available at the library are for individual use. They are not intended to be a resource for those in the tax preparation business.
Library staff members are not allowed to provide tax assistance or advise you on which forms you need.
